The following instrument, property of the Clarksville-Montgomery County School System, has been loaned to me for the 20 -20 school year. I/we agree to the terms and conditions set forth concerning the care, use and maintenance of this instrument. In the event of loss or damage, I/we agree to pay for the necessary repair and/or replacement of the instrument.

I, , accept full responsibility in event of any loss or damage resulting from my negligence to the school instrument listed below. I agree to keep the instrument in good playing condition and to pay for any repairs made necessary by misuse, negligence, or carelessness. I will present the instrument for inspection promptly when called for and will return in on or before May 11, 2002.
