I have read, understand, and agree to abide by all policies found therein during the period of my enrollment as a MSAT student at Waynesburg University. If I fail to uphold this agreement, I am aware that I may be disciplined in accordance with the MSAT program, Department, Graduate and Professional Studies, and University policies.

Further, I hold Waynesburg University’s administration and faculty harmless for any discussion of my academic and clinical progression and standing with clinical affiliate sites.
