The State of Ohio requires children entering kindergarten to have all of the required vaccinations. These include the DPT series as well as immunizations against measles, mumps, rubella, polio, Varicella (chicken pox) and Hepatitis B. Additionally, LSRCSS will only consider a registration form to be completed when it includes the student’s most recent transcript. Once all required documents have been gathered and handed in, the registration process will be initiated. Collection of a registration is not a guarantee of enrollment.
