You will need to fill out a FP-14 for all students that are parent, district to district, or state/court placed in our schools. These agreements are legal documents and it is very important to fill them out correctly. They have to be renewed EVERY year and you will be responsible for obtaining all the necessary signatures and making sure the document is filed with the appropriate state/county agency.
