THIS AGREEMENT is agreed to on the day of , 2012, by and between the City of Cleveland, Department of Public Utilities, Division of Water (“PURVEYOR”), under the authority of Ordinance No. 778-06, passed by the Council of the City of Cleveland on June 5, 2006 and Ordinance No. 1928-07, passed by the Council of the City of Cleveland on December 10, 2007, and the (“CITY”), under the authority of Ordinance No. 12-45.
