This Supply and Collaboration Agreement (the “Agreement”) is effective as of June 19, 2023 (the “Effective Date”), by and between DaVita Inc., a Delaware corporation, for the benefit of DaVita and its Affiliates (collectively referred to as “DaVita”), and Nuwellis, Inc., a Delaware corporation (“Nuwellis”). DaVita and Nuwellis may be referred to herein individually as a “Party” and collectively as the “Parties”.

This First Amendment to the Supply and Collaboration Agreement (the “First Amendment”) is effective as of May 31, 2024 (the “Effective Date”), by and between DaVita Inc., a Delaware corporation, for the benefit of DaVita and its Affiliates (collectively referred to as “DaVita”), and Nuwellis, Inc., a Delaware corporation (“Nuwellis”). DaVita and Nuwellis may be referred to herein individually as a “Party” and collectively as the “Parties”. Capitalized terms used but not otherwise defined herein shall have the meanings set forth in the Agreement.
SUPPLY AGREEMENT FOR CELLMID’S FGF5 INHIBITORSSupply and Collaboration Agreement • June 8th, 2021
Pursuant to the Agreement Advangen will manufacture and supply PUMP branded hair lotions utilising its proprietary FGF5 inhibitor hair loss prevention and hair growth technology. Pump will cover all marketing activities and will sell the hair lotions through its omnichannel distribution model. The two companies will also collaborate on new products; Cellmid will contribute its scientific expertise in developing anti-aging technologies and products and Pump will market these under its own brand.
