This Agreement to Provide Supported Living Services (this “Agreement”), dated as of July 1, 2024, is entered into by and between REGIONAL CENTER OF THE EAST BAY, INC., a California Non-Profit corporation located at 500 Davis Street, #100, San Leandro, CA 94577 (“RCEB”) and REVIVE SLS, LLC. a California Limited Liability company located at 675 Railroad Ave, Pittsburg, CA 94565 (“Contractor”), who mutually agree as follows:
