This UNE/Resale Forbearance Amendment(this “Amendment”), by and between Verizon Maryland LLC (“Verizon”), and Level 3 Communications LLC (“Customer”), upon execution by both Parties, shall be deemed effective as of the earlier of (i) the last date of signature by a Party and (ii) January 12, 2020. (Verizon and Customer may be hereinafter referred to individually as a “Party” and collectively as the "Parties"). This Amendment only covers the services addressed herein that Verizon provides in its operating territory in the State of Maryland (the “State”).

THIS AMENDMENT (this “Amendment”), effective as of January 12, 2020 (the “Effective Date”), amends each of the Interconnection Agreements (the “Interconnection Agreements”) by and between each of the Verizon incumbent local exchange carrier (“ILEC”) affiliates (individually and collectively “Verizon” or the “Verizon Parties”) and each of the “Bullseye Telecom” competitive local exchange carrier (“CLEC”) affiliates (individually and collectively Bullseye Telecom or the Bullseye Telecom Parties”; Verizon and Bullseye Telecom are referred to herein individually as a “Party” and collectively as the “Parties”). Attachment 1 hereto lists, to the best of the Parties’ knowledge, the Interconnection Agreements in effect between the Parties as of the Effective Date (the “Agreements”).

This Amendment No. 2 (the “Amendment”) is made by and between Verizon Maryland LLC, successor to Verizon Maryland Inc., (“Verizon”), a Maryland limited liability company with offices at 1 East Pratt Street, Baltimore, MD 21202, and Lumos Networks LLC, a West Virginia limited liability company with offices at One Lumos Plaza, Waynesboro, VA 22980 (“Lumos”), and shall be deemed effective on July31st , 2020 (the “Amendment Effective Date”). Verizon and Lumos are hereinafter referred to collectively as the “Parties” and individually as a “Party”. This Amendment covers services in Verizon’s service territory in the State of Maryland (the “State”).
