While the primary purpose of the Delano Union School District’s technology equipment is for use on site during the instructional day, there are times when it is appropriate for staff or students to check out equipment to be used for educational purposes beyond the school day. All users will be required to sign this form before technology equipment can be taken to a location other than a District facility. Technology equipment may be checked out under the provisions of the Acceptable Use Policy.

You and your student will comply with the Appropriate Use of Electronic Devices. You and your student agree that you and your student will review these policies and be familiar with their contents. Violations of the policy will result in revocation of network access privileges, suspension of access to SLUSD electronic resources, other school disciplinary action and/or other appropriate lethal or criminal action, including restitution. You must report any lost, stolen or damaged equipment to the school immediately. For stolen equipment, a police report is required.
