This document serves as a public offer to enter into an agreement for using the "MedCard AI" mobile application and providing medical consultations through telemedicine. The offer is made by the Medical Organization through the electronic platform operated by "MedCard AI" CJSC (hereinafter referred to as the Service Administrator) and is sent to an indefinite number of persons under the conditions described below.

MWR Life (“Company”) is headquartered at 20801 Biscayne Blvd, Suite 308, Aventura, FL 33180. All Members of this Service should read the terms of this Agreement carefully, and communicate any questions that arise to a service representative available by telephone Monday through Friday, from 9 a.m. to 5 p.m. ET at 1-888-536-1341.
