This Amendment to Contract No. 17-27-0006-M2, hereinafter referred to as “Tenth Eleventh Amendment” is made and entered into upon execution of all necessary signatures between the County of Orange, a political subdivision of the State of California, hereinafter referred to as “County” and Age Well Senior Services, Inc., DUNS # 792315459, a California non-profit corporation, with a place of business at 23101 Lake Center Drive, Suite 325; Lake Forest, CA 92630; hereinafter referred to as “Contractor,” with County and Contractor sometimes referred to as “Party,” or collectively as “Parties.”
