When value of shipment or released value of the entire lot or specific articles names exceeds sixty (60) cents per pound per article, appropriate insurance to cover the excess value, when requested by the shipper, will be arranged for by the carrier at shipper’s expense, and the charges therefore shall be added to the bill for transportation and any other lawful charges.

New York, NY 10007 (833) 985-1503 the Obligor of this Contract in all states except Florida. “New Leaf” refers to New Leaf Service Contracts, Inc., the Administrator of this Contract. “Seller” refers to Matrix Protection, the Seller of this Contract. “You”, “Your” and the “Customer” refer to the person contracting for Services covered by this Contract and whose name(s) appear on the Cover Page.
