The UNIVERSITY OF THE PHILIPPINES, the national university of the Philippines, created through Act No. 1870 and operating under Republic Act No. 9500, represented herein by the Chancellor of the University of the Philippines Manila, DR. MICHAEL L. TEE, with office address at the 8th Floor, RCB Building, Philippine General Hospital, Manila, hereinafter referred to as the “UNIVERSITY”;

For and in consideration of the training/education provided to the undersigned by the UNIVERSITY OF THE PHILIPPINES MANILA, the undersigned do hereby agree to assume all risks and be solely responsible for any injury, damage or loss sustained by the undersigned while participating in the traineeship/educational program for students which I am receiving credit from the UNIVERSITY unless such injury or loss arises solely out of the UNIVERSITY’s staff, employees and representative’s gross negligence or willful misconduct.
