On October 3, 2023, Sherri Millham, RSLP (the “Registrant”) entered into an Undertaking and Consent Agreement (the “Agreement”) following an investigation by the College’s Inquiry Committee (the “Committee”) into allegations that from July-August 2022, the Registrant failed to conduct an adequate Videofluroscopic Swallow Study (“VFSS”), made a clinically insupportable diagnosis based on the inadequate VFSS, failed to document important information on the VFSS report, and failed to provide follow-up care to the relevant patient following the VFSS.
