Nature of Action: The Registrant agreed to an Undertaking and Consent Agreement requested by the Inquiry Committee under section 36(1) of the Health Professions Act placing conditions on her practice. The Registrant agreed to meet with a supervisor selected by the College to review the following issues: (a) how the Registrant responds to requests for information from a child’s legal guardian; (b) the Registrant’s practice in obtaining informed consent when providing services to a parent and child, where there is another parent who is also a guardian; (c) the Registrant’s understanding of who is the client; (d) the Registrant’s regulatory obligations to respond to the College in a timely manner; and (e) the Registrant’s management of the impact of other obligations and circumstances on her professional responsibilities. On October 3, 2023, Sherri Millham, RSLP (the “Registrant”) entered into an Undertaking and Consent Agreement (the “Agreement”) following an investigation by the College’s Inquiry Committee (the “Committee”) into allegations that from July-August 2022, the Registrant failed to conduct an adequate Videofluroscopic Swallow Study (“VFSS”), made a clinically insupportable diagnosis based on the inadequate VFSS, failed to document important information on the VFSS report, and failed to provide follow-up care to the relevant patient following the VFSS.
