entered into by and between Catapult Health, LLC, on behalf of itself and its affiliated professional organizations (“Catapult”) and Unionville-Chadds Ford School District as plan administrator acting on behalf of its group health plan (“Customer”) and sets forth the terms and conditions under which Catapult will provide and/or make available the initial and periodic virtual preventive health and related services (the “Services”) to Customer’s employees, adult dependents and others designated by Customer, as applicable (collectively, “Participants”). This Agreement includes, and is subject to, the General Terms and Conditions, Business Associate Agreement, Exhibits and VirtualCheckup® Addendum, attached hereto.
