This UPMC Practice Solutions Participation Agreement sets forth the terms and conditions pursuant to which (the “Practice”), and the physician(s) listed on the signature page of this Agreement, as such listing may be amended from time to time, (the “Physician(s)”), agree to participate in the UPMC Practice Solutions program (the “Program”). By entering into this Agreement, the Practice, the Physician(s) and UPMC believe that they will advance their common objective of making high-quality, cost-effective health care services available to the Western Pennsylvania community.
