This Utilization Management Services Contract (Contract) is made by and between the Mississippi State and School Employees Health Insurance Management Board (Board), acting administratively through the Department of Finance and Administration (DFA), and ________________, (UM Vendor or Contractor) effective July 1, 2021, under the following terms and conditions under which the UM Vendor agrees to provide services to Board relating to the State and School Employees’ Health Insurance Plan (Plan), and any other plans/programs for which the Board is or becomes responsible during the term of this Contract.
