This agreement is for: Providers enrolling to receive state and/or federally funded flu vaccine for the influenza season 2020-2021 (e.g. college/universities, hospital emergency departments, COVID testing sites, etc.) Current MI-AVP providers do not need to complete this form as they are already eligible for adult flu vaccine this season. If your site is an FQHC, tribal health center or migrant health center, consider enrolling not as a flu-only provider but as a fulltime VFC and/or MI-AVP
