In exchange for participating in batting practice and related activities (“Batting Activities”) using the space and equipment (the “Facilities”) provided by Madtown Sluggers, Inc. I, request the use of the City of Waukegan’s (“City”) beach wheelchair (“Wheelchair”). I have read this Waiver and Indemnification Agreement (“Agreement”) and understand that by signing this Agreement, I am WAIVING and RELEASING, on behalf of myself, my child, the person of whom I have legal guardianship, my family, my household members and heirs, assigns, and legatees, all claims arising out of the use of the Wheelchair.
