Star Ready Athletes, doing business as Star Ready Athletes (“SRA”), is a sports performance, recruiting network, skills academy, sports and personal training organization located in Jacksonville, FL (“SRA”). I am executing this Waiver of Liability, Assumption of Risk, Release, and Indemnification Agreement (“Agreement”) in order to allow SRA to permit Participant’s use of said equipment and facilities, as well as participation in said activities and programs; and in consideration for SRA’s permission for Participant to utilize SRA’s personal training & sports performance facilities and/or equipment and to participate in SRA’s athletic activities and programs; and, willing to legally bind myself, Participant, and any other legal guardian or parent of the Participant and our personal representatives, and/or heirs. I do hereby acknowledge and agree to the following:
