I, (we) the undersigned (Applicant), hereby apply for water and/or sewer services from Powder Mountain Water and Sewer Improvement District. Applicant agrees to pay all monthly service charges required by the District for providing culinary water and/or sewer services to the property as listed above and to pay a late fee as set by the District on the entire unpaid balance of the account for the property to be served. Applicant also agrees that the culinary water and/or sewer service to the above property may be terminated of interrupted at or after the time of which the charges for wither become past due.
