Complete the chart below for all immediate family members to be included with your agreement. Immediate family members must reside with the primary applicant and be related as spouse/partner, sibling, grandchild, and/or one nanny or babysitter. Phone numbers and emails are for notifications of pool closures, re-openings, and related information. Originally issued FOBs will be used. There is a $15/FOB charge for any FOB replacements (old FOBs will be deactivated). FOB Application must be completed for both new residents and FOB replacements. FOBs are limited to 2 per household, no exceptions. PROOF OF RESIDENCY REQUIRED WHEN OBTAINING KEY FOBS.
