This WRITING PRODUCER AGREEMENT (the “Agreement”) is made by and between Metropolitan Life Insurance Company, an insurance company organized and existing under the laws of the state of New York with its principal office at 200 Park Avenue, New York, NY 10166-0188 (“MetLife”) and located at (“Writing Producer”). The Agreement governs the sale of MetLife Individual Disability Income Insurance policies through any of the general agents listed in Appendix A hereto (the “General Agent”) sold by: (i) the Writing Producer; and when the Writing Producer is an entity, rather than an individual
