In consideration of (Participant’s name) being allowed to participate in the youth water polo training, practices, scrimmages, games and tournaments of Carolina Water Polo, LLC ( the “Club”), its affiliates and leagues (together, the ‘Program’) as well as the use of any of the Facilities and the use of the equipment in connection with participation in the Program, as his/her custodial parent/guardian, I hereby agree as follows:
