Alternative Destination Partner definition

Alternative Destination Partner means an individual or entity that has agreed to furnish Covered Services, or to arrange for a Downstream Practitioner to furnish Covered Services, to an ET3 Model Beneficiary at an Alternative Destination owned or operated by the individual or entity following a Transport to an Alternative Destination intervention.

Examples of Alternative Destination Partner in a sentence

  • Transport a beneficiary to an Alternative Destination Partner such as an Urgent Care Clinic, Federally Qualified Health Center, or Community Mental Health Center (Transport to an Alternative Destination - TAD) [Note: transport to a primary care provider is disallowed], or2.
