Benefits for rehabilitation therapy are limited to services given for acute or recently acquired conditions that, in the judgment of the Insured's Provider and SHL’s Managed Care Program, are subject to significant improvement through Short-Term therapy. Covered Services do not include cardiac rehabilitation services provided on a non-monitored basis nor do they include treatment for intellectual disability. Covered Services include enteral formulas and special food product when prescribed by a Physician and authorized by SHL’s Managed Care Program for treatment of an inherited metabolic disease.

  • Medical Benefits means the monthly fair market value of benefits provided to the Employee and the Employee’s dependents under the major medical, dental and vision benefit plans sponsored and maintained by the Company, at the level of coverage in effect for such persons immediately prior to the Employee’s termination of employment date.
