Eye Examinations definition

Eye Examinations. Effective January 1, 2004, you can claim the cost of an eye examination to a maximum of $75 every two years.

  • Eye Examinations – a fifty dollar ($50.00) eye examination benefit which shall be available once in every twenty-four (24) consecutive month time period in accordance with the terms and conditions of the Plan.

  • Vision Care option (including Laser Eye Surgery and Eye Examinations) increased to $375 every 24 months.

Related to Eye Examinations

  • Medical examination means an in-person or virtual examination of a member’s physical or mental health, or both, by a physician or physicians selected or approved by the board; or, at the discretion of the board, a medical record review of the member’s physical or mental health, or both, by a physician selected or approved by the board.

  • Controller of Examinations means the Controller of Examinations of the University;

  • Due Diligence Examination shall have the meaning set forth in Section 3.2.

  • Initial Environmental Examination or “IEE” means the initial environmental examination for the Project, including any update thereto, prepared and submitted by the Borrower and cleared by ADB;

  • National examination means the examinations developed and administered by the Federation of State Boards of Physical Therapy and approved by the board for licensure as a physical therapist or physical therapist assistant.
