The Department will disenroll Members from a PH-MCO when there is a change in residence which places the Member outside the HC Zone(s) covered by this Agreement, as indicated on the individual county file maintained by the Department’s Office of Income Maintenance.

Note 3: It is understood that relocation benefits only apply to employees meeting the eligibility criteria as defined under the Employment Security and Income Maintenance provisions of Article 22.
The provisions of the Supplemental Agreement governing the Employment Security and Income Maintenance Plan will apply to employees covered by this Agreement as specified therein.
If a member asks the MCO to provide services during a temporary absence from its service area, the MCO shall conduct two tests to determine whether to provide the services: Request that Income Maintenance complete a residency test to determine whether the member is still considered a resident of a county within the MCO’s service area.
