Leave Requests. Employees who desire to use such leave for the birth or adoption of a child are required to apply in writing at least thirty (30) days in advance or within five (5) workdays of the onset of the leave in cases where the need for leave is unforeseeable.

Examples of Leave Requests in a sentence

  • Incidental Leave Requests will be granted if, in the opinion of the Employer, its operations will permit.

  • Except as otherwise provided in this Agreement, Leave Requests shall be subject to the approval of the Director or his/her designee.

  • Departmental recommendations regarding Educational Leave Requests shall be provided the employee within five (5) working days of the submission of such request.

  • Incidental Leave Requests may be granted in units of less than eight (8) hours if approved by the Employer.

  • Leave Requests Exceeding 14 Calendar Days shall require the approval of the Administrator/Controller or the respective Chief Judge and Human Resources Director if the employee is an employee of the courts.

  • Educational Leave Requests for Association business will be submitted to the current president of the Association prior to submission to the particular building administrator.

  • Department recommendations regarding such Education Leave Requests shall be provided to the employee within five (5) working days of the submission of such request.

  • Thus, leave will be scheduled and approved in advance through a Twelve (12) Month Annual Leave Schedule and through Incidental Leave Requests.

  • Leave Requests Leaves will be requested in writing as far in advance as possible and the Hospital will approve or deny such written request in writing as soon as possible.

  • Urgent care request means a claim relating to an admission, availability of care, continued stay or health care service for which the covered person received emergency services but has not been discharged from a facility, or any Pre-Service Claim or concurrent care claim for medical care or treatment for which application of the time periods for making a regular external review determination:

  • Release Request has the meaning set forth in Section 2.1.

  • New Service Request means an Interconnection Request, a Completed Application, or an Upgrade Request.

  • Service Request means a request for service such as adds, moves, changes and technical assistance;

  • Submitted Order has the meaning specified in Section 11.10(d)(i) below.
