Examples of Orthopedic Appliances in a sentence

  • The following Prosthesis and Orthopedic Appliances, if they satisfy Blue Cross Blue Shield of Wyoming’s Medical Policy and are otherwise Medically Necessary, are Covered Services, as well as fitting, adjusting, repairing, and replacement of an appliance due to wear, or a change in the Member’s condition which makes a new appliance necessary.

  • Replacement or repair of Durable Medical Equipment, Orthopedic Appliances, Prosthetic Appliances.

  • Purchase of Orthopedic Appliances or Prosthetic Appliances including, but not limited to, artificial limbs, eyes, larynx.

  • Replacement or repair of Durable Medical Equipment or Orthopedic Appliances.

  • IMPORTANT NOTE: Authorization by Blue Cross Blue Shield of Wyoming is required for certain Prosthesis and/or Orthopedic Appliances.

  • TMJ appliances are covered as set forth under Section IV.H.1., Orthopedic Appliances.

  • The failure to obtain authorization may result in a denial or reduction in coverage for the Prosthesis and/or Orthopedic Appliances.

  • Diagnostic Services means services provided for the purpose of determining the nature and cause of a condition, ill- ness, or injury.
