Peer Mentor definition

Peer Mentor means an individual who has been through the same or similar Recovery 19 process as those he/she is now assisting to attain their Recovery goals while getting paid for this 20 function by the program. A Peer Mentor practice is informed by his/her own lived experience with 21 behavioral health issues.
Peer Mentor is a person who is a current or former client of DDA who uses their lived experience to inform, support, and guide the Peer Mentee to successfully make the move from an ICF/IID to a home in the community. The Peer Mentor supports the Peer Mentee through the community transition process up to 60 calendar days after discharge from the ICF/IID and helps the Peer Mentee navigate new experiences related to the transition from the ICF/IID to the community.
Peer Mentor means a person with lived experence who has been trained to support those who struggle with mental health, psychological traua, or substance use.

  • The parties understand and agree that the eligibility of a family to receive Peer Support Specialists, Recovery Coaches, or Peer Mentor Services shall be determined by the Department.

  • The use or disclosure by the Contractor of information concerning an eligible parent, child(ren) or family in violation of any rule of confidentiality imposed by law or for any purpose not directly connected with the administration of the Department’s or the Contractor’s responsibilities with respect to the Peer Support Specialists, Recovery Coaches, or Peer Mentor Services hereunder is prohibited except on proper written consent of such eligible parent, his/her attorney or his/her responsible guardian.

  • The Contractor will be paid for all Peer Support Specialists, Recovery Coaches, or Peer Mentor Services rendered prior to receipt of said written notice.

  • When the Department has determined that the family is no longer eligible to receive Peer Support Specialists, Recovery Coaches, or Peer Mentor Services from the Contractor, the Department shall notify the Contractor in writing within 10 working days of this determination.
