Peer Mentor definition

Peer Mentor means an individual hired by the Contractor who has reached a high level 17 of recovery and has personal experience as well as work experience or training, in peer mentoring 18 activities such as linkage, bridging, and support.
Peer Mentor means an individual certified to provide peer support to a participant and assists a participant in navigating public behavioral health and medical services systems and the diversion court process.
Peer Mentor is a person who is a current or former client of DDA who uses their lived experience to inform, support, and guide the Peer Mentee to successfully make the move from an ICF/IID to a home in the community. The Peer Mentor supports the Peer Mentee through the community transition process up to 60 calendar days after discharge from the ICF/IID and helps the Peer Mentee navigate new experiences related to the transition from the ICF/IID to the community.

  • The Federation shall return to each teacher that teacher’s Faculty Self- evaluation form with concerns as may be noted by the Peer Mentor Committee.

  • The Peer Mentor Committee shall develop methods of evaluating the success of intervention strategies.

  • The Federation shall provide each teacher with a tabulation of the results of the teacher’s Student Evaluation of Faculty forms, copies of student comments appearing on the forms, and concerns that may be noted by the Peer Mentor Committee.

  • Only when a Peer Mentor Committee has determined, over the course of two consecutive semesters, that the completed forms of a particular teacher merit possible intervention shall the Committee be informed of the identity of the teacher in question.
