Physician service definition

Physician service means a medically necessary health service provided by or under the supervision of a physician.
Physician service means such services as are rendered by a licensed Physician within the scope of his license.
Physician service means medical assessment, treatments, and surgical services provided by licensed allopathic or osteopathic physicians within the scope of practice.

Physician service means a service provided within the scope of practice of medicine or osteopathy as defined by state law, by, or under the direction of a person licensed under state law to practice medicine or osteopathy.
Physician service means those services which a Participating Physician agrees to make available to Beneficiaries of Qualified Health Plans, when such services are covered under the Health Plan, and such services are required by a Beneficiary’s medical condition. Participating Physicians agree to provide medical care and surgical care as appropriate to Beneficiaries within the parameters of their practice or specialty and within existing office schedules and physician settings which they customarily provide for all patients.
