Reportable Incident definition

Reportable Incident means a situation in which a Client has suffered Harm or experienced a Close Call or any Hazard that could lead to Client harm, all in accordance with Schedule “D”, Appendix 4.
Reportable Incident means the following conduct that a mandated reporter is required to report to the vulnerable persons' central register:
Reportable Incident means any accident, injury, property or environmental damage or any potential breach of any Law that occurs during or as a result of this agreement. Schedule refers to the Schedule of this agreement.

Examples of Reportable Incident in a sentence

  • There is a difference of almost half a pH unit between the lowest measurement (7am) and the highest (2pm), and measurements can change rapidly as demonstrated by the increase between 11am and 12 noon.

  • If a Reportable Incident occurs while an Apprentice is conducting activities identified in their Apprentice Progress Review or which are relevant to the MA Programme, you must ensure we are informed immediately by contacting your Skills Investment Advisor (SIA) by phone (leaving a message containing brief details, if unanswered) and email.

  • Post Reportable Incident Testing will take place no later than 12 hours after the incident where a reportable incident falls within the following criteria.

  • Directors of mental health providers or their designees must ensure that their incident management programs require any staff person who observes or is informed that a Reportable Incident of any type has occurred to immediately provide assistance and secure appropriate care for the involved patient or patients.

  • Oil spills that occur inside machinery spaces with no potential to harm the environment shall be treated as part of an equipment failure or related to the creation of a hazardous or unsafe condition and reported in accordance with the definition of a Reportable Incident above.


More Definitions of Reportable Incident

Reportable Incident means an event described by one or more of the following:
Reportable Incident means an act by a health care provider which: (1) Is or may be below the applicable standard of care and has a reasonable probability of causing injury to a patient; or (2) may be grounds for disciplinary action by the appropriate licensing agency.
