Service Record definition
Service Record means the documentation, written or electronic, regarding an individual and resulting from entry, assessment, orientation, services and supports planning, services and supports provided, and transfer.
Service Record means an individual, tangible, and legible file that records service-related activities set forth in subrule 152.2(4).
