Access to Individuals. Participant #2 agrees to provide Individuals with access to their Protected Health Information, as held in a Designated Record Set by Participant #2, in order to meet the requirements under 45 CFR § 164.524, including in the electronic form or format requested by the Individual, as required by 45 CFR § 164.524. In the event any individual requests access to Protected Health Information directly from Participant #2, it shall forward such request to Participant #1.
