Access to Participating Provider Locations. Participating Provider locations shall be confirmed by the FIDA Plan through site visits to be accessible for Participants with disabilities. The FIDA Plan shall collect sufficient information from Participating Providers to assess compliance with the ADA. Compliance shall be confirmed through Provider attestation to the ADA Accessibility Attestation Form and a random sampling of on-site compliance review throughout the Demonstration. As necessary to serve Participants, Participating Provider locations where Participants receive services shall be ADA compliant. All Participating Providers must meet ADA requirements and have a signed ADA Accessibility Attestation Form on file with the FIDA Plan per Section 1.8.
