ACCOUNTING OF DISCLOSURES OF PROTECTED HEALTH INFORMATION. a. Disclosure Records: Business Associate will keep a record of any Disclosure of PHI that Business Associate makes, if Covered Entity would be required to provide an accounting to Individuals of such Disclosures under 45 C.F.R. § 164.528. Business Associate will maintain its record of such Disclosures for six (6) years from the termination of this Agreement.
