Active enrollment. By signing this Agreement, the Provider agrees Provider is available and able to provide services to one or more Recipients who are eligible for publicly-funded in-home services in Oregon. This Agreement will be inactivated if services are not authorized or paid during a twelve-month period. Following inactivation, the Provider may reapply for enrollment as a PSW if Provider wants to provide services to DHS Recipients.

Active enrollment. You must maintain an active System profile in order to continue enrollment in eStatements. To remain active, you must maintain a valid user ID and password and access the System at least once every three hundred and sixty-five (365) days. If you fail to maintain an active System profile or a valid email address, your enrollment in eStatements will be discontinued and you will start receiving paper copies of Communications.
Active enrollment. An enrollment with status of enrolled. Drops and never attends are excluded. Only active enrollments can receive a course grade.

The Contractor shall require his/her/its subcontractors, who perform work under this Contract, to certify to the Contractor that the subcontractor does not knowingly employ or contract with an unauthorized alien and that the subcontractor has enrolled and is participating in the E-Verify program. The Contractor agrees to maintain this certification throughout the duration of the term of a contract with a subcontractor. The State may terminate for default if the Contractor fails to cure a breach of this provision no later than thirty (30) days after being notified by the State.
