Addition of Documentation of the Evaluation. Process A copy of the recommendation made at each step of the evaluation process shall be added to the portfolio. If an Employee has requested reconsideration of a negative recommendation by a DPC, Division/ Department Chair or Xxxx, if applicable per Article 18.4, or the UPC, a copy of the written statement of the result of the reconsideration shall be included in the portfolio. A copy of all written evaluations placed in an Employee's evaluation portfolio shall be provided to the Employee in a reasonable time.
