Common use of Additional Claim Information Clause in Contracts

Additional Claim Information. 1.4.1. The Claims Administrator or the Special Master may require such additional records or other documentation (including further documentation) as either of them may determine is material and necessary (i) to determine whether a particular Enrolled Program Claimant meets the Eligibility Requirements or (ii) for purposes of the Claims Valuation Process (any such further required records or other documentation, the “Additional Claim Information” ). In such cases, the Claims Administrator or the Special Master shall issue a written request to the Enrolled Program Claimant’s Counsel, or if without counsel, to the Enrolled Program Claimant. 1.4.2. An Enrolled Program Claimant must produce Additional Claim Information requested pursuant to Section 1.4.1 either within 60 days of service of such request or by the deadline set forth in Section 1.3.1, whichever is later. 1.4.3. Additional Claim Information shall be submitted by means of a Supplementary Claims Form executed and delivered as specified in Section 1.3.2.
