Advanced Medical Homes. For all contracts with any provider who is an Advanced Medical Home (AMH), a provision that outlines the AMH care management model and requirements consistent with the Department’s Advanced Medical Home Program. Each contract with an AMH shall include a statement that the contracted provider agrees to comply with the Department’s Advanced Medical Home Program.
