Care Coordination Services. Care coordination was previously referred to as “case management.” DHCS has retitled and re-described this benefit as “care coordination.” Care coordination shall be provided to a client in conjunction with all levels of treatment. Care coordination consists of activities to provide coordination of SUD care, mental health care, and medical care, and to support the beneficiary with linkages to services and supports designed to restore the beneficiary to their best possible functional level. Care coordination can be provided in clinical or non-clinical settings (including the community) and can be provided face-to-face, by telehealth, or by telephone. Care coordination includes one or more of the following components:
Care Coordination Services. BHS shall assign a Care Coordinator to each Participant request. The Care Coordinator will serve as an advocate for the Participant and will provide support and follow-up to ensure the Participant is connected to the right resources and is making progress. The Care Coordinator will also collect and review Participant satisfaction and outcomes.
Care Coordination Services. Participating Provider shall provide primary care and patient care coordination services to each Covered Person, in accordance with Carolina Complete policies.
