AHS Use. The programs in section A-1 and A-2 where there is an “A” in the column entitled “AHS Use” are limited-use licenses that may only be used solely for the Vermont Agency of Human Services and the following AHS departments (collectively, the “Authorized Users of the AHS Use programs”): the Department of Mental Health (DMH); the Department of Disabilities, Aging and Independent Living (DAIL); the Department for Children and Families (DCF); the Department of Vermont Health Access (DVHA); the Department of Health (VDH); and the Department of Corrections (DOC). You warrant and represent that you have the full legal authority to bind the Authorized Users of the AHS Use programs to the terms and conditions of this ordering document and agreement, and that you shall be responsible for any breach by such Authorized Users of the AHS Use programs.”

AHS Use. The programs in section A-1 where there is an “A” in the column entitled “AHS Use” are limited-use licenses that may only be used solely for the Vermont Service Oriented Architecture – Infrastructure Components Project to support AHS’ technical infrastructure and business applications in order to support the provision of services for Vermont’s Human Services functions by you and the following AHS departments (collectively, the “Authorized Users of the AHS Use programs”): the Department of Mental Health (DMH); the Department of Disabilities, Aging and Independent Living (DAIL); the Department for Children and Families (DCF); the Department of Vermont Health Access (DVHA); the Department of Health (VDH); and the Department of Corrections (DOC). You warrant and represent that you have the full legal authority to bind the Authorized Users of the AHS Use programs to the terms and conditions of this ordering document and agreement, and that you shall be responsible for any breach by such Authorized Users of the AHS Use programs.”

  • Business Use Coverage is provided if the BUSINESS USE surcharge has been paid as specified on YOUR DECLARATION PAGE. Use the format provided by the CAM. • Prepare a Final Project Fact Sheet at the project’s conclusion that discusses results. Use the format provided by the CAM. • Prepare a Technology/Knowledge Transfer Plan that includes: o An explanation of how the knowledge gained from the project will be made available to the public, including the targeted market sector and potential outreach to end users, utilities, regulatory agencies, and others.

  • Substance Use Disorder counseling shall be provided by a QCC, or Chemical Dependency Counselor Intern. Substance use disorder education and life skills training shall be provided by counselors or individuals who have been trained in the education. All counselor interns shall work under the direct supervision of a QCC.

  • Other Uses a. entering into a license agreement between and among BCBSA, the Plan and a debit card issuer located outside the Plan’s Service Area, and entering into a corresponding operating agreement or agreements, in order to offer a debit card bearing the Licensed Marks and Name to eligible persons as defined by the aforementioned license agreement;

  • Third Party Use You may allow your agents, contractors and outsourcing service providers (each a “Permitted Third Party”) to use the Product(s) licensed to you hereunder solely for your benefit in accordance with the terms of this XXXX and you are responsible for any such Permitted Third Party’s compliance with this XXXX in such use. Any breach by any Permitted Third Party of the terms of this XXXX will be considered your breach.
