Annual Employee Performance Evaluation. Each permanent, full-time and permanent, part-time Employee's job performance shall be reviewed annually and a written performance rating prepared during the month of May. The written performance evaluation shall be reviewed and signed by the Employee and the supervisor before it is sent to the Human Resources Director or her/his designee. A copy will be provided to the Employee. An Employee desiring to challenge a performance evaluation rating may, at the time the Employee signs the copy, indicate that the Employee desires to submit a written response which must be submitted within fourteen
