Appearance Agreement. I understand that SA may from time to time produce promotional and other audio or visual materials and media relating to the Activities (the “Promotional Material”). I understand that, as a participant in or spectator of such Activities, Minor may be included in video recordings, audio recordings, photographs, and images taken during Activities for use in Promotional Material. THEREFORE, I HEREBY ASSIGN, TRANSFER, AND GRANT TO SA, WITHOUT RESERVATION OR LIMITATION, THE ROYALTY-FREE, EXCLUSIVE RIGHT TO USE SUCH VIDEO RECORDINGS, AUDIO RECORDINGS, PHOTGRAPHS, AND IMAGES OF MINOR, AS WELL AS MINOR’S NAME, LIKENESS, PERSONAL AND DEMOGRPAHIC INFORMATION, VOICE AND APPEARANCE AS PART OF ANY PROMOTIONAL MATERIAL. IN GRANTING THIS LICENSE, I HEREBY WAIVE ANY RIGHT TO INSPECT OR APPROVE ANY SUCH PROMOTIONAL MATERIAL PRIOR TO PUBLICATION. I FURTHER AGREE THAT, UPON THE REQUEST OF SA, I SHALL IMMEDIATELY REMOVE OR TAKE DOWN ANY PROMOTIONAL MATERIALS THAT MINOR OR I HAVE UPLOADED OR POSTED TO ANY WEBSITE OR SOCIAL MEDIA PLATFORM (INCLUDING, BUT NOT LIMITED TO, FACEBOOK, TWITTER, INSTAGRAM AND YOUTUBE), OR IN ANY OTHER PUBLIC FORUM.
Appearance Agreement. I understand that Adrenaline from time to time produces promotional material relating to its programs and Activities. I understand that as a Participant and/ or a spectator at an Activity that I or the Participant may be included in video, audio and photographs taken during the Activities. Therefore, without reservation or limitations, I, in my own behalf and on behalf of the Participant, hereby assign, transfer and grant to Adrenaline, its successors, assignees, licensees and third parties the right to photograph, audio and / or video myself of the Participant and to utilize such audio, video and photographs and my or Participant’s name, face likeness, voice and appearance as a part of the Activities, in advertising and promoting Adrenaline. I, in my own behalf and on behalf of the Participant, waive any right to inspect or approve the copies of any promotional materials related thereto.

I understand that as a participant and/ or a spectator at the Event that Minor may be included in videotapes or photographs taken during the Event. I understand that Superior Festivals from time to time disclose participants information to third parties who agree to the confidential nature of the information. Event Rules. I further acknowledge and understand that Superior Festivals has established rules and regulations pertaining to conduct, behavior and activities of all Event participants, by which ▇▇▇▇▇ and I agree to abide during the Event (copy of which is listed on the back of this form), and that Minor and I will be responsible for his/her/my failure to abide by those rules and regulations. Minor and I have received, read and understand the Event rules. Minor and I understand that violation of the rules can result in dismissal from Event with no refund.

DDA has a class dress code and performance appearance standards by which we expect ALL students to apply in order to maintain a professional, distraction free environment for everyone. By signing this form, you agree to follow the dress code for classes and to wear the proper shoes, tights, costuming, and follow hair and make-up requirements for all performances.
