Attachment A01 - Performance. Contract Notebook, Version 2 is amended and restated with Attachment A01 - Performance Contract Notebook, Version 3, which includes, but is not limited to, the following substantive modifications: a. Revisions documenting the shift of administrative rules from Title 25 of the Texas Administrative Code, to Title 26 of the Texas Administrative Code; b. Addition of a requirement to notify System Agency when Contractor experiences phone system technical issues or a service disruption exceeding 48 hours; c. Addition of a requirement to dedicate at least one full-time staff member to support continuity of care and related functions; d. Addition of a requirement to make a staff member available for consultation with a state mental health facility regarding use of atypical long-acting injectable antipsychotic medication upon discharge; e. Expansion of the number of Adult Needs and Strengths Assessments (ANSAs) and Children’s Needs and Strengths Assessments and (CANS) Super Users to two per assessment tool; f. Designation of one or more staff members to track and disseminate System Agency policy updates internally; g. Expansion of the age limitations for enrollment in Transition Age Youth Services; and h. Expansion of Contractor’s coordination responsibilities when children or youth are enrolled in the System Agency-funded Residential Treatment Center program.
