Benefits Available. District shall provide health, dental, life and income protection benefits to unit members and their families. 7.1.1 The District shall provide all members with a choice between Kaiser and SISC/Blue Shield. All employees will be provided with information regarding costs, co-pays, hospital stays, and prescriptions. Open enrollment is during the month of August with an effective date of the following October. The medical benefits cap shall be $548.00 per month. The cap shall be prorated for part-time employees on the basis that the number of hours worked bears to 2080 hours. 7.1.2 The dental plan shall be Delta Dental which will be equal to or better than the current dental plan. 7.1.3 The life insurance plan shall be in the amount currently provided. 7.1.4 The income protection plan shall be as currently provided.

Benefits Available. Prescription drugs other than contraceptives are covered up to a thirty (30) day supply (“Standard Supply”). You may get two vacation supplies per Benefit Year. This means that you may get up to an additional 30 day supply for vacation periods two times per Benefit Year. You must pay the applicable multiple payments for a vacation supply. Prescription contraceptive drugs and devices approved by the FDA are covered subject to any applicable Cost Share as per HRSA guidelines, and may be covered up to a 12 month supply. MVP will permit prescriptions to be filled by such retail pharmacy in the same manner and at the same level of reimbursement as they are filled by mail order pharmacies with respect to the quantity of drugs or days’ supply of drugs dispensed under each prescription.
