Board's Obligation. a. The Board shall provide a teacher with an application or enrolment form for participation in the medical, dental, extended health and group life insurance benefit plans. In the event a teacher does not wish to participate in any particular benefit plan where opting out is an option, the application or enrolment form must so be noted by the teacher and kept on file by the Board.
