Care and Dental Care Benefits for Incapacitated Children. Health Care and Dental Care Benefits will continue beyond the date an unmarried child attains the limiting age for insurance, provided proof is submitted to Excelsior Life within 31 days after such date that such child: . is incapable of self-sustaining employment by reason of mental retardation or physical handicap; . became so incapacitated prior to attainment of the limiting age; and . is chiefly dependent upon you for support and maintenance. Thereafter, such proof must be submitted to Excelsior Life, as required, but not more often than yearly.
