Dental Care Plan. The Welfare Plan will include a Dental Care Plan which will reimburse members for expenses incurred in respect of the coverages summarized in Appendix "1". The Plan will not duplicate benefits provided now or which may be provided in the future by any government program.
Dental Care Plan. The Company will provide a dental plan, with participation compulsory for all employees, on the following basis:
Dental Care Plan. 8.01 The Agency will continue to offer coverage to employees under the Dental Care Plan as contained in the agreement between the Treasury Board and the Public Service Alliance of Canada, as amended from time to time by the terms and conditions of the Dental Care Plan Agreement between the Public Service Alliance of Canada and the Treasury Board.
Dental Care Plan. 1. It is agreed that the State shall continue the Dental Care Plan during the period of this Agreement. The Dental Care Plan may be modified by the State Health Benefits Plan Design Committee, pursuant to its authority under P.L. 2011, c. 78. Pursuant to P.L. 2011, c. 78, the State Health Benefits Plan Design Committee has the sole discretion to set the amounts for maximums, co-pays, deductibles, and other such participant costs for all plans in the program and has the sole discretion to determine the plan design, plan components and coverage levels under the program. Full-time employees and eligible dependents shall be eligible for the State administered Employee Dental Plan(s).
2. Participation in the Plan shall be voluntary with a condition of participation being that each participating employee authorize a biweekly salary deduction as set by the State Health Benefits Plan Design Committee.
3. A member handbook describing the details of the Plan, enrollment information and the required enrollment forms are available on the Division of Pensions and Benefits’ website.
4. Participating employees shall be provided with an identification card to be utilized when covered dental care is required.
Dental Care Plan. 7.01 The Public Service Dental Care Plan, as amended from time to time by the terms and conditions of the Dental Care Agreement between the Treasury Board of Canada Secretariat and the Public Service Alliance of Canada shall be deemed to form part of this Agreement.
Dental Care Plan a) Plan “A” (diagnostic and preventive services, restorative services, surgical services, prosthetic repairs): 100% reimbursement by the plan.
b) Plan "B" Prosthetic Appliances and crown and bridge procedures: 50% reimbursement by the plan.
c) Plan "C" Orthodontics: available to employee and dependents only after patient has been covered continuously for 12 months, maximum benefits of $2,500 with 50% reimbursement by the plan.
Dental Care Plan. The Welfare Plan will include a Dental Care Plan which will reimburse members for expenses incurred on the following general principles:
1) Basic Dental Services (Plan A) - Plan pays 90% of approved schedule of fees.
2) Prosthetics, crowns and bridges (Plan B) - Plan pays 50% of approved schedule of fees.
3) Orthodontic (Plan C) - Plan pays 50% of approved schedule of fees. The maximum lifetime benefit is $5,000 per person for all services provided by an Orthodontist. The Plan will not duplicate benefits provided now or which may be provided in the future by any government programs.
Dental Care Plan. 8.01 The Dental Care plan as contained in the Master Agreement between the Treasury Board and the Public Service Alliance of Canada, with an expiry date of June 30, 1988, and subsequently amended on March 10, 1988, December 12, 1991, November 26, 1993, April 2, 1996, January 15, 1997 and March 11, 1998 shall be deemed to form part of this Agreement.
Dental Care Plan. (a) Plans A and B, Basic Services - diagnostic, preventive, surgical services, etc.; • 100% reimbursement from the Plan.
(b) Plan C, Prosthetic Appliances and crown and bridge procedures; • 50% co-insurance.
(c) Plan D, Orthodontics - available to employees and dependents only after patient has been covered continuously for 12 months, maximum lifetime benefits $2,500 per patient. • 50 % co-insurance.
